Requests to return material for repair or credit must be in writing using the RMA form and must be pre-approved by Code Blue prior to return of material. Code Blue will review the request, verify that all the required information has been submitted and then fax a numbered return authorization document that is to be used as a packing slip. The RMA number must be on the outside of the carton to be accepted upon return to Code Blue. ANY RETURNS THAT DO NOT HAVE A PRE-APPROVED RMA NUMBER NOTED ON EACH BOX RETURNED WILL BE REJECTED.

Approved RMA's are valid for 30 days from issue date and material must be returned within this time frame.

RMA Process

Call Customer Service or Tech Support at 800-205-7186 for pre-approval.  At this time Code Blue will attempt to troubleshoot your issue over the phone.  If the situation cannot be resolved over the phone you will be given pre-approval for an RMA request to be generated.

Fill out the RMA request form on your computer, print the form (all required fields must be filled in or your RMA form will not print) and FAX it to the number shown.  Please note: Hand written RMA requests will not be accepted.

The following information is REQUIRED when filling out an RMA request:

  • A Purchase Order Number for any repair charges and return freight
  • Type of Return (use the drop box for selection)
  • Bill To and Ship To information
  • Phone and Fax numbers
  • Original Code Blue Sales Order or Invoice Number
  • End User Name
  • Item Being Returned (use drop box for item selection), Quantity and Serial Number
  • NOTE: For warranty consideration, include Proof of Purchase such as Sales Order, Purchase Order or Order Number

Shipping Instruction

  1. Please pack the item in its original packing if possible to protect against rough handling during transportation. You are responsible for this product until we receive it. If you do not have the original packing, please ensure the item is properly protected.
  2. You are responsible for the shipping and insurance charges to ship the item to Code Blue.
  3. Enclose a completed copy of the RMA form along with your proof of purchase in the package.
  4. Be sure the RMA # is on the outside of the package (use the faxed authorization document as a shipping label)

Item Review

All RMA's are examined within 7 business days after receipt. Material returned for repair under warranty will be repaired or replaced with like item and returned to you. If your item is not under warranty, you will be notified of the cost of services needed to complete the repair. There is a minimum charge of $37.50 MSRP for all non-warranty inspections or repairs.  Repair approval is required against the purchase order provided on the RMA before repairs can be made. All service items are covered by a 90-day limited warranty after the service date or the remaining original warranty period whichever is longer.
